As part of Adult Social Care’s Integration Agenda an opportunity has arisen for a Social Worker to become part of North Neighbourhood Hub. This role sits within a multi-disciplinary team and provides social work support and intervention.
You will be based at Moorpark health Center alongside a very supportive team of health staff including GP’s, nurses, wellbeing workers and therapists. You will have links to daily MDT meetings which have links to wider health teams such as CPN’s and dieticians. You will work autonomously in arranging your day to day work. As such experience and knowledge of all associated Legislation will be needed to carry our your role.
Tasks will include the completion of Care Act Assessments and Reviews, consideration of CHC requirements, Safeguarding work and crisis interventions. A foundation of the role will be your ability to form and maintain relationships with colleagues and negotiate person centered outcomes from a strength based approach to social care. You will be part of a wider Adult Social Care Team for day to day support and supervision/appraisal.
