    Salary: Grade H4 SCP 41 – 46 £47,573 – £52,674 per annum

    Blackpool’s Integrated Community Learning Disability Team has an exciting opportunity for experienced practitioner from either social care or health to be the new Integrated Team Manager for the Community Learning Disability Team.
     
    The team comprises of adult social care staff, from Blackpool Council and NHS health staff from Blackpool Teaching Hospitals. We are looking to recruit to the post of Integrated Team Manager at H4/Band8a to provide leadership and direction to the integrated team.
     
    The post holder will be expected to meet service delivery working Monday to Thursday 9am – 5pm, Friday 9am – 4.30pm.
     
    We are looking for an enthusiastic and committed practitioner to lead and manage. You will have knowledge of commissioning, social care, NHS health assessments for people with learning disabilities, legal frameworks and an awareness of the emerging influences in both health and social care.

    You will have excellent communication and organisational skills, competent knowledge of the implementation of the Care Act 2014, experience of managing and leading on safeguarding, managing risk and care management, understanding of the NHS Long Term Plan, the health inequalities that affect people with learning disabilities and key themes / learning from LeDeR and the transforming care agenda.

    In your position as Team Manager you will offer leadership to a team of social workers, Learning Disability nurses, other health professionals and allied health professionals in the service delivery of the integrated team across the Blackpool footprint. You will understand the value and importance of integrated working with partner agencies in delivering support, protection and developing independence to individuals utilising a strength based approach to carers and their families. You will be providing management support and guidance in relation to complex learning disabilities and complex health needs ensuring all operational pathways for health and social care are followed and maintained to provide an exceptional service offer to the people of Blackpool.

    The post holder will assist the Head of service for Mental Health and Learning Disability (Blackpool Teaching Hospitals) and the Service Delivery Manager for Learning Disability and Autism Services (Blackpool Council) in ensuring there is leadership within the team actively promoting teamwork whilst supporting the team members via supervision and appraisals. The post holder will have direct line management of the deputy team managers for social care and the health team. There will be a requirement to undertake managerial functions of the team and escalate matters of concern to relevant senior managers.
