We have an exciting opportunity for a part time (18.5 hours) Social Worker to join the Transfer of Care Hub Social Worker team.
The Transfer of Care Hub is an integrated and health and social care team co-located at Blackpool Teaching Hospitals to support people to be discharged in a safe and timely manner.
We are looking for a practitioner with experience and interest in crisis intervention and safeguarding; who is able to make robust evidence based and legally literate decisions to support people and their families navigate hospital discharge and access onward care and support services in the community.
You will be required to apply a strengths – led approach to care and support planning, ensuring the person’s voice is heard; and be passionate about supporting Blackpool residents.
You will have access to an established group of managers with the knowledge and experience needed to support you in this role. You will have access to regular supervision and training opportunities are available to continue to fulfil your continuous professional development needs.
You will work as part of a 7 day rota which is determined with advanced notice and paid at an additional 2 increments.
