If you would like to work within a challenging and supportive environment, Blackpool Council is pleased to be able to offer a social work vacancy within the Community Mental Health Team for Older Adults.
We are offering an exiting opportunity to work in the Older Adult CMHT in Blackpool. We are a co-located team working with older adults with various mental health issues and their carers. Working alongside our health colleagues in an integrated way of working that is well established within the team.
We are committed to a joint approach to service planning, delivery and development.
For Older Adults, you will provide multi-disciplinary needs and risk assessments, care planning and provide long term social work support within the framework of the Care Act 2014 and in line with the national agenda driving the provision of care to older adults with a mental health problem.
You will work within the framework of the Care Act 2014 and undertake timely assessments and reviews.
We welcome applications from both experienced and newly qualified Social Workers.
Approved Mental Health Professional status would be advantageous, but not essential.
