We have an exciting opportunity for a qualified Social Worker based at Trinity Hospice for 18.5 hours per week.
The successful candidate will work alongside Trinity Hospice inpatient and community Multi- Disciplinary teams to provide timely and robust care and support to individuals and families in need.
You will need to empathetic and compassionate; and have a passion for supporting individuals and families in end of life care planning.
You will be part of a wider health and community social care team and will have access to a supportive and enthusiastic team members and managers; and will have access to regular supervision and team development opportunities.
You will be supporting Blackpool, Fylde and Wyre residents and will be required to work with Lancashire County Council colleagues, managers and use LAS recording systems.
